Plane leak was maintenance fault

A series of maintenance errors led to a fuel leak as a British Airways plane took off from London's Heathrow Airport, says an accident report.

The leak on the Boeing 777 aircraft bound for Zimbabwe was caused by fuel escaping through an open door removed during maintenance but not fixed back.

The Air Accidents Investigation Branch (AAIB) are attaching some of the blame to BA's Cardiff-based maintenance team.
